Mayor\u2019s Permit.<\/strong> All persons are required to obtain a Mayor\u2019s Permit for the privilege, of&nbsp;conducting business within the city.<\/p>\n<p><strong>TAX ORDINANCE NO. 012-2019,CHAPTER III, Article A, Section 84. A Mayor\u2019s Permit shall not be issued to:<\/strong><\/p>\n<ol>\n<li>Any person who previously violated any ordinance or regulation governing permits granted.<\/li>\n<li>Any person whose business establishment or undertaking does not conform to zoning regulations, and safety, health and other requirements of the city.<\/li>\n<li>Any person who has unsettled tax obligation, debt or other liability to the government.<\/li>\n<li>Any person who is disqualified under any provision of law or ordinance to establish or operate the business applied for.<\/li>\n<\/ol>\n<p><strong>TAX ORDINANCE NO. 012-2019,CHAPTER III, Article A, Section 84. Rules and Regulations on Certain Establishments.<\/strong><\/p>\n<p>a. On restaurants, cafes, cafeterias, carinderias, eateries, food caterers, ice cream and other refreshment parlors, soda fountain bars. No owner of said establishments shall employ any cook, or food dispenser without a Food Handler\u2019s Certificate from the City Health Officer, renewable every six (6) months.<\/p>\n<p>b. Establishments selling cooked and readily edible foods shall have them adequately covered and protected from dust, flies and other insects, and shall follow strictly the rules and regulations on sanitation promulgated by the City health Officer and existing laws or ordinances.<\/p>\n<p>c. Sauna bath, massage, barber and beauty shops. Said shops shall not be allowed to operate with masseurs, barbers and beauticians without having secured the necessary corresponding medical certificate from the City Health Officer. &#8216;<\/p>\n<p><strong>TAX ORDINANCE NO. 012-2019,CHAPTER III,&nbsp; Article C. Building Permit<\/strong><\/p>\n<p>Section 94. Imposition of Fee. There shall be collected from each applicant for a building permit fees prescribed in the National Building Code, as amended.<br>Section 95. Time and Payment. The fees specified under this Article shall be paid to the City<br>Treasurer upon application for a building permit from the City Mayor.<br>Section 96. Administrative Provisions. The application for the construction and or repair shall be in writing and shall set forth the required information, such as the location and the general dimension of the building and\/or other infrastructure of the owners as well as that of the architect or engineer who draw the plan, an estimate of the entire cost of proposed work, and the following:<\/p>\n<p>A copy of the plan showing the location of the building to be constructed with the reference boundaries of the lot and is constructed in the town proper or poblacion.<br>b. General drawing showing:<\/p>\n<ol>\n<li>Floor and roof plans<\/li>\n<li>Foundation and footing plans<\/li>\n<li>Transferee and longitudinal plan<\/li>\n<li>Elevation<\/li>\n<li>Framing plans showing complete forming of the building or structure<\/li>\n<li>Isometric view of plumbing layout<\/li>\n<li>Electrical layout<\/li>\n<li>Detail of structure and architectural parts<\/li>\n<\/ol>\n<p><strong>TAX ORDINANCE NO. 012-2019,CHAPTER III, Article D. Permit Fee for Zoning\/Locational Clearance<\/strong><br>Section 98. Imposition of Fee. There shall be collected a Zoning\/Locational Clearance for all structures constructed in this city<\/p>\n<p><strong>TAX ORDINANCE NO. 012-2019,CHAPTER III,<\/strong> <strong>Article I. Permit Fee on Occupation of Calling Not Requiring Government Examination<\/strong><br>Section 118. Imposition of Fee. There shall be collected as annual fee at the rate prescribed hereunder the issuance of mayor\u2019s Permit to every person who shall be engaged in the practice of the occupation or calling not requiring government examinations, with the city<\/p>\n<p><strong>TAX ORDINANCE NO. 012-2019,CHAPTER III, Article N. Permit Fee for the Storage of Flammable and Combustible Materials<\/strong><br>Section 140. Imposition of Fee. There shall be collected an annual permit fee for the storage of combustible materials<\/p>\n<p><strong>TAX ORDINANCE NO. 012-2019,CHAPTER III,<\/strong> <strong>Article O. Fire Inspection Fees<\/strong><br>Section 143. Imposition of Fees. There shall be imposed an Annual Fire Inspection Fee on any person, natural or juridical, who uses or occupies any building or structure in the City of Ilagan<\/p>\n<p><strong>TAX ORDINANCE NO. 012-2019,CHAPTER III, Article E. Sanitary Inspection Fee<\/strong><br>Section 182. Imposition of Fees. There shall be collected the following annual fees from all business establishment in this city or house for rent, for the purpose of supervision and enforcement of existing rules and regulations and safety of the public<\/p>\n<p><strong>TAX ORDINANCE NO. 012-2019,CHAPTER III, Article F. Service Fees for Health Examination<\/strong><br>Section 185. Imposition of Fee. There will be collected a fee of Two Hundred Pesos (P200.00) from any person who is given a physical examination by the City health Officer of his duly authorized representative, as required by existing ordinances.<\/p>\n<p>Section 187. Administrative Provisions.<br>a. Individuals engaged in an occupation or working in the following establishments are hereby required to undergo physical and medical examination before they can be employed and once every six (6) months thereafter.<\/p>\n<ol>\n<li>Food establishments \u2013 establishments where food or drinks are manufactured, processed, stored, sold or served.<\/li>\n<li>Public swimming or bathing places.<\/li>\n<li>Dance schools, dance halls and night clubs \u2013 include dance instructors, hostess, cooks, bartenders, waitresses, etc.<\/li>\n<li>Tonsorial and beauty establishments \u2013 include employees of barber shops, beauty parlors, hairdressing, tattoo shops and manicuring establishments, exercise gyms and figure slenderizing saloons, facial centers, aromatherapy establishments, etc.<\/li>\n<li>Massage clinics and sauna bath establishments \u2013 include masseurs, massage clinic\/sauna bath attendants, etc.<\/li>\n<li>Hotels, motels, and apartments, lodging, boarding or tenement houses, and condominiums.<\/li>\n<\/ol>\n<p>b. Owners, managers or operators of the establishments shall see to it that their employees who are required to undergo physical and medical examinations have been issued the necessary medical certificates.<\/p>\n<p>c. The City Health Officer shall keep a record of physical and other health examinations conducted, and the copies of medical certificates issued indicating the name of the applicant, the date and the purpose for which the examination was made<\/p>\n<p><strong>TAX ORDINANCE NO. 012-2019,CHAPTER III, Article G. Environmental Fee<\/strong><br>Section 189. Imposition of Fee.
